Health & Housing Co-ordinator
We are currently partnering with a supported housing service based in West Yorkshire, they are seeking an experienced Support Worker for a 2 month, temporary role. The role is full time Monday to Friday and requires candidates to hold an Enhanced DBS and experience of providing support to vulnerable clients. A car would be essential as the role requires travel between hospitals and clients' houses. Please find below a summary of the duties: In this fast-paced environment you will find, investigate and provide resolutions to issues preventing a patient from going home safely You will at times be required to provide discharge support to associated cottage hospitals, step down bed facilities and other acute hospitals in Kent The role will also require you to undertake home visits as necessary, actively assessing the safety and suitability of the home environment Working with the patient, family or carers, support appropriate and timely discharge of the patient back into the community through necessary interventions, these may include: working with Local Authority partners, internal services, community facilities and support services Home visits are required post-discharge to assess the home environment and support independent living Ensure accurate records both paper and electronic are maintained to evidence compliance with the service specification Attend meetings as required reviewing, checking and updating the progress of cases ensuring that Peabodys service is well represented Liaise promptly with colleagues, partner agencies and other stakeholders regarding safeguarding, welfare and risk concerns in accordance with Peabody policy
