We provide high quality, responsive, OT Assessment Services within a case management model. Our Adult and Social Care Services and Concessionary Travel are expanding and constantly evolving to ensure we achieve the highest standard of assessment and clinical evaluation. Our objective is to provide clear, clinical assessments for Adult Social Care Contracts in response to the increasing demand for specialist assessment services. We provide services for and Adult and Social Care, Blue Badge, Travel Pass, Taxicard and Parking Bays Overall purpose of the post: To provide a high-quality Occupational Therapy assessment of adults and children with physical, sensory and learning disabilities. To make recommendations to improve the safety and independence of those people in their own homes Assessments to include provision of equipment, minor and major adaptations, Moving and Handling, Housing Needs and Concessionary Travel. To provide documentation to support those recommendations and any specifications, plans, diagrams to enable case progression Main responsibilities and duties: To confirm annual leave, at least a month in advance, unless urgent annual leave is necessary and agreed by a manager. To receive referrals from Able 2 administration team and undertake a comprehensive assessment of need within the individual service user's home or other prearranged location, preparing for visits by reading all background documentation provided. To complete the relevant assessment form and care plan detailing all the findings of the assessment confirming and clinically justifying the recommended services in accordance with relevant eligibility criteria. To record and report any concerns regarding clients, being particularly vigilant regarding Safeguarding and Mental Capacity issues. Depending on Contract requirements, directly order equipment and minor adaptations recommended, or provide specific details enabling them to be arranged by the administrative team via equipment suppliers. To provide functional specifications and drawings for major adaptations within the client's home. To provide a full Moving and Handling Risk assessment and Moving and Handling plan. To forward the completed assessment reports to Able 2 for processing within required time scales, following the assessment visit as per contract and KPI's. Unless for valid reasons, non-conformance will result in small fines as per our contract procedures. To undertake follow up visits, check for any other issues at property and complete the relevant paperwork. To work in case management and support complex cases as per contract requirements. To attend case conferences and panel meetings or provide thorough written clinical summary of case prior to the meeting. OT must facilitate joint working with MDT members for complex cases as required. To attend Ombudsman/legal proceedings as necessary. To be available for regular supervision, annual appraisal and discussion of complex cases when required with a Supervisor/Occupational Therapy Manager. Company expectation is that you attend meetings and training sessions provided/organised by Able 2/NRS. To be available for responding to case queries as required within procedural/contractual timescales. To undertake joint visits with specialist equipment representatives/surveyors when necessary, providing the most suitable provision for the client. To ensure that all duties carried out for Able 2 are undertaken in a timely, professional and appropriate manner. (full stop) To understand and comply with all relevant policies and procedures. To comply with Royal College of Occupational Therapists (RCOT) and Health & Care Professions Council (HCPC) Codes of Practice and professional regulations. Person Specification: Education and Training: To hold a recognised Occupational Therapy Professional Qualification To be registered with the Health & Care Professions Council Experience: To have 5 years post qualification experience of working in a local authority community setting Knowledge/ Technical skills To have a good understanding of a range of conditions and resultant disabling effects To have excellent assessment skills To have extensive knowledge of the range of specialist equipment and adaptations that can promote independence To have the ability to write specifications and draw diagrams for major and minor adaptations The ability to build upon previous assessments and progress cases in a timely manner To be IT literate and able to record and communicate electronically via relevant database/systems To keep up-to-date with legislation pertaining to relevant contract To commit to the development of the company Personal Skills: To demonstrate excellent communication skills To be reliable and responsive throughout the client journey To show strong commitment to problem solving to bring about positive outcomes for clients To be aware and actively responsive to issues relating to mental capacity and safeguarding Additional requirements: Professional Indemnity Insurance Cover with a minimum value of £5 million.
